When is this ceremony preformed?
It really depends on the wedding, there is no right answer, however most do it following the toasts, invocation, and meal, but before the first dance of the evening!
Which layer should they cut?
The caterer and/or photographer should make that decision. Definitley not the top layer!

After the bride & groom cut the cake, what next?
After the bride and groom have completed their cake cutting ceremony, the couple has the option of serving to their new in-laws. The rest of the cake is cut by the catering staff or friend designated in advance and served to all of the wedding guests.

Two Traditions
#1) Have the top layer of the cake made out of fruit cake or cheesecake. This layer is saved and frozen for the bride and groom to eat on their first anniversary. (Wrap this very carefully-and designate someone to be in charge of getting this layer to you.)
#2) A piece of the bride's cake under a single person's pillow will lead to "dreams of a future spouse", according to tradition. Have the caterer pack slices in decorative boxes to send home with guests.
